Abstract

ABSTRACT Adherence to antiretroviral (ARV) medication in HIV/AIDS patients is a crucial aspect of disease management and is essential for achieving viral suppression and improving quality of life. A major challenge is maintaining patient adherence to long-term treatment regimens. Telenursing with the use of mobile phone reminder has been an innovative approach in improving patient adherence to ARV medication. This systematic review aims to review and evaluate the effectiveness of telenursing with mobile phone reminder on ARV medication adherence in HIV/AIDS patients. This study is a systematic review with literature search using 5 databases namely ProQuest, PubMed, Science Direct, Scopus, and Embase published from 2015 to 2023 with RCT design. The keywords in the search were Telenursing AND Mobile Phone Reminder AND HIV OR AIDS OR PLWHA AND Medication AND Adherence AND Antiretroviral Therapy (ART). Quality assessment of the selected articles used the Joanna Briggs Institute (JBI). There are ten articles showing the effectiveness of using telenursing: mobile phone reminder on ARV medication adherence. Telenursing: mobile phone reminder is an effective intervention to improve ARV medication adherence among PLWHA. Implementation of this intervention can help PLWHA achieve viral suppression and improve quality of life. Keywords: Telenursing, Mobile Phone Reminder, Adherence, Medication, Antiretroviral, HIV/AIDS/PLWHA  ABSTRAK Kepatuhan minum obat antiretroviral (ARV) pada pasien HIV/AIDS (ODHA) merupakan aspek krusial dalam pengelolaan penyakit dan sangat penting untuk mencapai supresi virus serta meningkatkan kualitas hidup. Tantangan utama yang dihadapi adalah menjaga kepatuhan pasien terhadap regimen pengobatan jangka panjang. Telenursing dengan penggunaan mobile phone reminder telah menjadi pendekatan inovatif dalam meningkatkan kepatuhan pasien terhadap minum obat ARV. Systematic review ini bertujuan untuk meninjau dan mengevaluasi efektivitas telenursing dengan mobile phone reminder terhadap kepatuhan minum obat ARV pada pasien HIV/AIDS. Studi ini adalah tinjauan sistematis dengan pencarian literatur menggunakan 5 database yaitu ProQuest, PubMed, Science Direct, Scopus, dan Embase yang diterbitkan dari tahun 2016 sampai dengan 2023 dengan desain RCT. Kata kunci dalam pencarian adalah Telenursing AND Mobile Phone Reminder AND HIV OR AIDS OR PLWHA AND Medication AND Adherence AND Antiretroviral Therapy (ART). Penilaian kualitas artikel yang dipilih menggunakan Joanna Briggs Institute (JBI). Terdapat lima belas artikel yang menunjukkan efektivitas penggunaan telenursing: mobile phone reminder terhadap kepatuhan minum obat ARV. Telenursing: mobile phone reminder merupakan intervensi yang efektif untuk meningkatkan kepatuhan minum obat ARV pada ODHA. Abstract

ABSTRACT HIV-AIDS is a disease that currently cannot be cured. Until now, no drug has been found can cure HIV, but antiretroviral drugs (ARV) have proven to be very effective in delaying the emergence of AIDS by forming antibodies and producing pro-inflammatory agents to suppress the number of viruses in the blood so that can inhibit damage to immune system (Permenkes, 2016). Adherence to taking ARV is very important to be maintained because they must be taken throughout life (Kemenkes Ditjen P2P, 2024). To analyze factors that influence adherence to antiretroviral (ARV) medication in HIV-AIDS patients, through a systematic review. The design of this research is a literature review using the PRISMA flow diagram. Researcher used PEO and searched for English language articles, which is published from 2018 to March 2024. The databases used were Scopus, Pubmed, Science Direct, EBSCO and Proquest. Beside of the five databases, the researcher added two Indonesian language articles related to the purpose of the literature, which were the result of lecturers of Nursing Science study program, Faculty of Nursing, University of Indonesia. Researcher uses the Covidence tool for the data selection process through to extraction. Sixteen articles were identified and analyzed. Ten of the sixteen articles had cross-sectional study design. The sample size of the studies ranged from 9 to 792 which were conducted in various geographical areas, 50% of the articles, which are 8 articles was carried out in country on the African continent, most of them were South Africa, East Africa and West Africa and the remaining articles came from the American continent as well as Southeast Asia and South Asia. From the literature review conducted by the researcher, the researcher concluded several factors that influence ART treatment adherence which include factors related to the patient himself (gender or sex, age, level of education, socio-economic status, self-awareness, alcohol drinking habits and drug use), stigma (openness about illness, disclosure of status to partner), social support (partner, family, social environment), treatment (side effects of medication and duration of HIV illness), health services (convenience and trust in health facilities). Keywords: HIV, AIDS, Factors Influence, Medication, Adherence  ABSTRAK HIV-AIDS merupakan penyakit yang saat ini belum dapat disembuhkan. Hingga saat ini, memang belum ditemukan obat yang dapat menyembuhkan HIV, tetapi obat antiretroviral (ARV) terbukti sangat efektif dalam menunda munculnya AIDS dengan membentuk antibodi dan menghasilkan agen proinflamasi untuk menekan jumlah virus dalam darah sehingga dapat menghambat kerusakan sistem kekebalan tubuh (Permenkes, 2016). Kepatuhan minum ARV sangat penting untuk dipertahankan karena harus diminum selama hidupnya (Kemenkes Ditjen P2P, 2024). Melakukan analisis faktor-faktor yang mempengaruhi kepatuhan minum obat Antiretroviral (ARV) pada pasien HIV-AIDS, melalui tinjuan sistematik. Desain penelitian ini adalah literature review menggunakan diagram alur PRISMA. Peneliti menggunakan PEO dan mencari artikel bahasa Inggris yang diterbitkan dari tahun 2018 sampai dengan Maret 2024. Database yang digunakan meliputi Scopus, Pubmed, Science Direct, EBSCO dan Proquest. Selain dari kelima database tersebut peneliti menambahkan dua artikel berbahasa Indonesia terkait tujuan literature, yang merupakan karya dosen, program studi Ilmu Keperawatan, Fakultas Ilmu Keperawatan Universitas Indonesia. Peneliti memanfaatkan tool Covidence yang dilakukan untuk proses seleksi data sampai dengan ekstraksi. Enam belas artikel diidentifikasi dan dianalisis. Sepuluh dari enam belas artikel memiliki desain cross sectional studi. Ukuran sampel dari studi berkisar dari 9 sampai dengan 792 yang dilakukan di berbagi wilayah geografis, sebanyak 50% yaitu 8 artikel, penelitiannya dilakukan pada negara bagian dari benua Afrika, yang terbanyak adalah Afrika Selatan, kemudian Afrika Timur dan Afrika Barat dan sisanya artikel berasal dari benua Amerika serta Asia Tenggara dan Asia Selatan. Dari literature review yang dilakukan peneliti, peneliti  menyimpulkan beberapa faktor yang mempengaruhi kepatuhan pengobatan ART yang meliputi faktor yang berhubungan dengan pasien sendiri (gender atau jenis kelamin, usia, tingkat pendidikan, status sosial ekonomi, kesadaran diri, kebiasaan minum alkohol dan penggunaan narkoba), stigma (keterbukaan tentang penyakit, pengungkapan status kepada pasangan), dukungan sosial (pasangan, keluarga, lingkungan sosial), pengobatan (efek samping obat dan lama sakit HIV), pelayanan kesehatan (kemudahan dan kepercayaan terhadap fasilitas kesehatan). Kata Kunci: HIV, AIDS, Factors Influence, Medication, Adherence

Abstract

Sifon is a post-circumcision ritual of sexual intercourse in the Atoin Meto tribe with women who are not their partners. Sifon women have various reasons for doing this practice such as fulfilling their economic and daily needs. Most of Sifon women are the heads of families. In addition to having an impact on the risk of contracting HIV, AIDS and STD Infection, Sifon also has other impacts on women such as social stigma and shameful accusation. Objective:The purpose of this study is to explore the psychological experiences of Sifon women in Timor Island.Method:This study used a phenomenological approach and purposive sampling technique. Participants were with inclusion criteria. They have to be more than 18 years old and have practiced Sifon in the last 6 (six) months. Data collection was carried out from March to July 2020 in Kupang Regency and South Central Timor Regency, East Nusa Tenggara Province. The data was processed using Collaizi method.Results: There were 13 women who were successfully interviewed and 7 of them shared their psychological experiences. Most of them are farmers, have elementary school education and become the head of their families. Based on the results of data analysis, the researcher found 3 themes: (1) feeling shameful, (2) feeling humiliated and not worthed, (3) having trauma.Conclusions:The decision to become a Sifon woman was made with various considerations, including meeting economic needs and earming money for all family members. It is necessary to take a culturally sensitive approach and deeper psychological assistance with Sifon women to help improve their wellbeing. The findings of this study show that most of these Sifon women are heads of families.
