Abstract

AbstrakEkspresi transforming growth factor beta 1 pada luka diabetes melitus mengalami penurunan yang berdampak terhadap proses penyembuhan luka. Penelitian ini bertujuan untuk membandingkan perawatan luka modern dressing dengan metode konvensional terhadap ekspresi transforming growth factor beta 1 pada luka kaki diabetes melitus. Penelitian menggunakan quasi experimental pretest-posttest design dengan metode pengumpulan sampel secara consecutive sampling. Pengukuran ekspresi transforming growth factor beta 1 dilakukan pada hari ke-0 (pretest) dan ke-4 (posttest). Hasil penelitian didapatkan data pada kelompok modern terjadi peningkatan ekspresi transforming growth factor beta 1, sedangkan pada kelompok konvensional terjadi penurunan ekspresi transforming growth factor beta 1. Disimpulkan bahwa teknik perawatan luka secara modern mampu meningkatkan ekspresi transforming growth factor beta 1dibandingkan teknik konvensional pada luka kaki diabetes melitus. AbstractReduction of expression of transforming growth factor beta 1 in diabetic ulcers affects overall wound healing. This study tried to draw a comparison of transforming growth factor beta 1 level between modern dressing and conventional dressing in diabetic foot ulcer. This study applied a quasi-experimental pretest-posttest design and a consecutive sampling method of data collection. Immunohistochemical analysis of transforming growth factor beta 1 level was measured on the day 0 (pretest) and the day 4 (posttest). In this study, the modern dressing application improves transforming growth factor beta 1 level. Meanwhile, the conventional dressing application decreases transforming growth factor beta 1 level. Thus, it can be concluded that the modern dressing application can increase transforming growth factor beta 1 level.

Abstract

AbstrakPenelitian ini merupakan penelitian kualitatif untuk menggali pengalaman partisipan mencari makna hidup. Partisipan penelitian berjumlah delapan parisipan dengan purposif sample. Pengalaman partisipan memberikan gambaran secara utuh diketahuinya perasaan partisipan dihadapi lupus dan perubahan nilai, kepercayaan, dan keyakinan penderita lupus. Hasil penelitian yang diperoleh mendapatkan respon penderita menghadapi penyakit lupus terhadap aktivitas fisik, psikologis, dan perubahan lingkungan serta perubahan nilai, kepercayaan dan keyakinan penderita lupus. Hasil penelitian menyarankan tenaga perawatan melakukan asuhan yang komprehensif, untuk tenaga pendidikan juga harus memberikan prosedur melakukan asuhan keperawatan holistik kepada peserta didik. Untuk peneliti lain agar menambah variasi jenis kelamin partisipan agar memperoleh variasi tema. AbstractThis research is a qualitative study to explore the experiences of participants to find meaning in life. Amount of research participants with a purposive sample of eight paricipants. The Experience gives participants how to life with lupus and how to find changes in value, trust, and confidence in people with lupus. The results are getting face lupus patients response to physical activity, psychological, and environmental changes and changes in values, trust, and confidence in people with lupus. The results suggest that nurse must do comprehensive care, to education personnel must also provide procedure to conduct a holistic nursing care to students. For another researchers in order to increase the participants’ gender variations in order to obtain variation on the theme.

Abstract

AbstrakPenelitian kuasi eksperimen ini bertujuan menguji pengaruh cold pressor test (CPT) terhadap pasokan dan kebutuhan oksigen miokard pada perokok aktif dengan pengukuran tekanan darah dan segmen ST di Kecamatan Ungaran Kabupaten Semarang. Jumlah sampel dalam penelitian ini sebanyak 76 perokok aktif di Kecamatan Ungaran Kabupaten Semarang dan ditentukan dengan cluster sampling. Pengaruh CPT terhadap tekanan darah dan segmen ST pada perokok aktif diuji dengan dependent sample t test. Hasil penelitian menunjukkan rerata tekanan darah setelah intervensi CPT mengalami penurunan. Hasil analisis dengan uji t test menunjukkan bahwa CPT menurunkan tekanan darah setelah intervensi CPT (p=0,000). Rata-rata segmen ST sebelum dan setelah intervensi CPT tidak jauh berbeda. Hasil uji t test menunjukkan bahwa CPT tidak berpengaruh dalam menurunkan segmen ST pada perokok aktif (p = 0,895). Penelitian ini menyimpulkan bahwa CPT berpengaruh terhadap perubahan tekanan darah pada perokok aktif tetapi tidak berpengaruh terhadap perubahan segmen ST. Selama CPT terjadi peningkatan tekanan darah dan berangsur menurun setelah dilakukan CPT. Segmen ST tidak mengalami perubahan. CPT dapat digunakan oleh perawat untuk memprediksi kejadian peningkatan tekanan darah. Untuk penelitian selanjutnya sebaiknya digunakan alat ukur yang lebih sensitif dan menggunakan determinan pasokan dan kebutuhan oksigen miokard yang lain seperti preload dan denyut jantung. AbstractThis quasy-experimental research was aimed to examine the effect of cold pressor test (CPT) to the myocardial oxygen demand and supply among active smokers in the Ungaran district, Semarang by measuring blood pressure and ST segment. There were 76 active smoker males involved as the cluster sampling in the research. To examine the CPT effect on the blood pressure and ST segment among the active smokers, dependent sample t test was conducted. The result of the research showed that average blood pressure is decreased after CPT. Furthermore, CPT showed its effects on decreasing blood pressure after the intervention (p = 0,000). The averages of ST segment before and after intervention of CPT were slightly different. Thus, CPT indicated no significant impact in decreasing ST segment among active smokers (p = 0,895). The research concluded that CPT caused blood pressure changes among active smokers but do not have effect in ST segment changes. On the other hand, it was observed that the initial blood pressure were increase during CPT but then gradually decreased immediately after the end of CPT. In addition, ST segment remained unchanged. Therefore, the CPT is still beneficial for predicting high blood pressure in patient compared to other cardiac stress test. Nevertheless, it is recommended to use more sensitive device and to consider other determinants of the myocardial oxygen demand and supply such as preload and heart beat for further research.

Abstract

AbstrakKaki diabetik merupakan komplikasi yang sangat menakutkan bagi pasien DM karena risiko amputasi yang sangat tinggi. Tujuan penelitian ini adalah untuk menganalisis faktor-faktor yang berkontribusi terhadap kejadian kaki diabetik pada pasien DM. Penelitian ini merupakan penelitian deskriptif analitik dengan pengumpulan data secara cross sectional. Sampel sebanyak 136 pasien DM yang mempunyai kelainan bentuk kaki dan luka kaki. Hasil analisis bivariat menunjukkan bahwa ada hubungan senam kaki, perawatan kaki, kepatuhan dalam pencegahan luka, kontrol gula darah, pengetahuan, dan diet dengan kejadian kaki diabetik. Analisis multivariat menunjukkan hanya ada 3 variabel yang merupakan faktor paling berkontribusi yaitu senam kaki, kepatuhan dalam pencegahan luka, dan pengetahuan. Rekomendasi hasil penelitian ini adalah melakukan konseling dan senam diabetes secara rutin. AbstractFoot problems in diabetes is one the most frightening complications for diabetic patients because of the possibility of amputation. The purpose of this study was to analyze the factors which contribute to foot problems. This research was an analytic descriptive research with cross sectional method design. The sample used for this study was 136 diabetic patients who suffered from foot problems or deformities. The result from bivariate analysis showed that there was a relationship between foot exercise, foot care, compliance in preventing foot injury, control of blood glucose level, patient’s knowledge, and diet with the incidence of foot problems. Whilst the multivariate analysis showed there were only 3 variables which contributed to this incidence. They were foot exercise, compliance in preventing foot injury and patient’s knowledge. This study recommended to have a routine counseling and diabetic exercise regulary.

Abstract

AbstrakPenelitian ini bertujuan mengidentifikasi perbedaan keefektifan penyembuhan luka menggunakan balutan madu dan balutan normal salin-povidone iodine pada pasien trauma dengan luka terbuka yang dirawat di salah satu RS di Bukittinggi. Desain penelitian ini adalah kuasi eksperimen, non-equivalent control group dengan pre dan post-test. Sampel berjumlah 6 responden (3 responden untuk masing-masing kelompok intervensi madu serta normal salin-povidone iodine). Hasil penelitian menunjukkan tidak ada perbedaan bermakna pada rerata skor perkembangan proses penyembuhan luka antara sebelum dan sesudah intervensi perawatan luka dengan madu (P = 0.076) dan dengan normal salin-povidone iodine (P = 0,057). Rerata skor perkembangan penyembuhan luka terbuka setelah intervensi tidak berbeda secara signifikan (P = 0,797) antara kelompok intervensi dengan madu dengan kelompok kontrol. Namun, penurunan skor perkembangan proses penyembuhan luka pada balutan madu (11,52%) lebih besar 6,67% dibandingkan balutan normal salin-povidone iodine (4,85%). Perawatan luka dengan madu membuat responden tidak merasa nyeri, tidak terjadi perlengketan serta perdarahan saat membuka balutan ketika dibersihkan, sedangkan dengan normal salin-povidone iodine, responden merasakan sebaliknya. Hasil penelitian ini merekomendasikan penggunaan balutan madu untuk pasien dengan luka terbuka. AbstractThe aim of this study was to compare the effectiveness of honey dressing and normal salin-povidone iodine dressing in the open wound healing process at a hospital in Bukittinggi. This was a non-equivalent control group quasi experimental study with pre & post test. The samples of this study were 6 respondents (3 respondents in each intervention and control group). The finding from this study showed that there was no significant difference on the mean score of wound healing process before and after wound care intervention using honey dressing (P = 0.076), and normal saline-povidone iodine dressing (P = 0.057). There was also no significant difference on the mean score of wound healing process on traumatic open wound patient after intervention on the control group using normal saline-povidone iodine dressing and intervention group using honey dressing (P = 0,797) However, the wound healing score on the honey intervention group was 6,67% higher (11,52%) than on the wound using normal saline-povidone iodine dressing (4,85%). Unlike patients in the control group, patients using honey dressing were not complaining about pain and bleeding when change dressing. Therefore, the study recommended the honey application for open wound.

Abstract

risk  walking  exercise  merupakan  salah  satu  bentuk  moderate  aerobic  exercise  yang direkomendasikan oleh ahli jantung di Amerika dan Eropa sebagai salah satu perubahan  gaya  hidup pasien hipertensi. Penelitian ini bertujuan untuk  mengetahui adanya pengaruh brisk walking exerciseterhadap  tekanan darah pasien hipertensi  di Kudus. Penelitian dilakukan pada  penderita hipertensi  di unit rawat jalan dua rumah sakit di Kudus dengan metode  penelitian eksperimen randomized control trial (RCT) dengan pendekatan pre dan post with control. Penelitian dilakukan pada  42 responden ( 21 responden kelompok kontrol dan 21 kelompok intervensi). Hasil uji paired t test perubahan tekanan darah sistolik dan diastolik kelompok kontrol menunjukan adanya nilai yang bermakna (p= 0,000 dan p=  0,026;   α  =  0,05).  Untuk  itu  perlu  adanya  penerapan brisk  walking  untuk  penatalaksanaan hipertensi di rumah sakit maupun puskesmas (komunitas).

Abstract

Penelitian fenomenologis ini telah mengkaji aspek bio-psiko-sosio-spiritual klien pengidap kanker payudara yang tengah mendapatkan pengobatan. Data kualitatif dalam bentuk catatan lapangan telah dikumpulkan selama pelaksanaan program kelompok pendukung. Maksud penelitian ini adalah untuk mendapatkan pemahaman yang mendalam tentang bagaimana wanita Indonesia yang mengidap kanker payudara merasakan pengalaman yang kompleks, sangat interpersonal, dan dinamis. Latar belakang pemikiran dari penelitian ini didasari oleh pengertian bahwa untuk mendapatkan pengetahuan tentang sesuatu akan menjadi sia-sia dan hamper tidak mungkin tanpa menerangkan pengalaman individu sebagaimana adanya ketika dialami dan sebagaimana diterangkan oleh individu itu secara langsung (Polit, 1996). Tujuan penelitian ini adalah untuk mengkaji dampak kanker payudara dan pengobatannya terhadap aspek (a) fisiologis, (b) keseimbangan psikologis, (c) hubungan klien dengan orang lain, (d) nilai-nilai spiritual, dan (e) kualitas kehidupan keseharian klien. Subyek penelitian terdiri dari 87 wanita dengan diagnosa kanker payudara yang menghadiri kelompok pendukung. Hasil analisa kualitatif berasal dari analisis isi komentar dan pernyataan subyek selama mengikuti kegiatan kelompok pendukung telah mendapatkan lima kategori thema yaitu: ketidak-mampuan fisiologis (physiological incapability), ketidak-seimbangan psikologis (psychological disequilibrium), perilaku negatif dalam hubungan social (social relationship misbehavior), disparitas nilai-nilai spiritual )spiritual values disparity), dan kehidupan bersemangat (life of courage). Setiap penyataan dinilai dan diinterpretasikan menjadi thema, kelompok thema, kategori thema. Kesimpulan hasil riset telah menunjukkan bahwa partisipasi kelompok dalam kelompok pendukung telah menolong mereka untuk membentuk perasaan positif tentang diri mereka sendiri dan meningkatkan kemampuan mereka dalam berkomunikasi dengan pasangan hidup dan anggota keluarga lainnya. This phenemonological study explored bio psycho social spiritual aspects of Indonesian women with breast cancer who were having therapy. A qualitative data in a form of field notes was obtained during the course of the support group program. The main objective was to have a deeper and richer understanding on how Indonesian women with breast cancer made sense of an experience that was complex, interpersonal, and dynamic. The premise was that gaining knowledge would be impossible without describing human experience as it was defined by the persons themselves (Polit, 1996). The purpose of the study was to explore the impact of breast cancer and its treatment on their (a) physiological aspects, (b) physiological equilibrium, (c) relationship with others, (d) spiritual values, and quality of daily living. Eighty seven subject who joined the support group for women with breast cancer participated in the study. The result of qualitative analysis which were extracted from the significant statements and comments made by the subject during the course of the breast cancer support group for women with breast cancer participated in the study. The result of qualitative analysis which were extracted from the significant statements and comments made by the subject during the course of the breast cancer support group program identified five theme categories : physiological incapability, physiological disequilibrium, social relationship misbehavior, spiritual values disparity, and life of courage. Each statement was examined and interpreted in relation to themes, theme group, and theme categories. The result of the study concluded that group participation had helped them to have positive feelings about themselves and improved their communication skills with their spouses and other family members.

Abstract

Adolescence is a period of transition from children to adult. The young undergo metamorphosis physical impact for the progress of the psychological result in adolescents at risk of experiencing health problems. A problem that is often experienced in adolescents between the other Drug abuse. Research objectives to get an overview of the effectiveness of Peer Teenagers Healthy (TEBARS) in the prevention of Drug abuse in adolescents in school, using a quasi-experiment, the sample obtained by random. The results illustrate Peers Teenagers Healthy is quite effective in preventing Drug abuse on teenagers in school, lack of knowledge of the average of 28.83 be 27.59, SD 1.558, p=0.000, risky behavior average of 49.23 be 43.32, SD 4.365, p=0.000. The average appearance Peers Teenagers Healthy before the intervention of 5.3 points increased to 7.8 point. The results of this study are expected to be implemented in schools and developed and fostered more to prevent Drug abuse.

Abstract

Introduction: Stroke is a condition of brain function loss due to disturbance in cerebral blood flow that occurs more than 24 hours. The difficulties and dysfunction are caused by brain damage entail long-term disorders of physical and mental balance, so that the patients depend on their families. Objective: The aim of this study was to assess the impact of the family empowerment program on the functional status of patients after stroke and also family preparedness to taking care the patients at home. Method: The study design was a quasi-experiment design with pretest-posttest control group approach using 25 respondents. Groups were divided into a control group (n=12) and intervention group (n=13). Results: The results showed that is no significant difference between functional status in both groups after the intervention, but there is a significant difference in family preparedness to taking care for stroke survivors between the control and intervention groups (p = 0.004 at α = 0.05). Conculsion: Based on the results, the provision of family empowerment program as a preparation for discharge planning could be one of the nursing interventions for families to giving a care for stroke survivors at home

Abstract

AbstrakTujuan penelitian ini adalah mengidentifikasi persepsi pasien HIV/AIDS dan keluarganya tentang HIV/AIDS dan stigma masyarakat terhadap pasien HIV/AIDS. Pendekatan yang digunakan pada penelitian kualitatif ini adalah fenomenologi. Sejumlah 13 informan (7 pasien dan 6 anggota keluarga) berpartisipasi dalam penelitian ini. Profil demografik informan klien HIV/AIDS meliputi pendidikan sekolah menengah atas sampai perguruan tinggi, berjenis kelamin pria usia 18–40 tahun. Informan keluarga umumnya adalah kakak kandung (berjenis kelamin pria dan wanita) dan orangtua (ibu) dari klien HIV/AIDS berusia 45–55 tahun. Hasil analisis didapatkan beberapa tema: (1) Pengetahuan pasien/keluarga tentang HIV/AIDS yang rendah (2) Pasien tidak terinformasi tentang penyakitnya (3) Kepercayaan yang salah tentang HIV/AIDS dan cara penularannya (4) Pasien HIV/AIDS bertanggung jawab atas dan pantas untuk terinfeksi HIV/AIDS (5) Perasaan takut dikucilkan/ upaya untuk merahasiakan tentang penyakitnya (6) Perasaan tersisih/ dikucilkan oleh keluarga dan kelompok tertentu (7) Intimidasi/ teror fisik (8) Putus asa dan (9) Harapan pasien HIV/AIDS. Hasil penelitian ini dapat bermanfaat untuk meminimalkan stigma dengan cara mengoptimalkan pengetahuan masyarakat sehingga deteksi dini pada orang yang berisiko dapat dilakukan. Hal ini memungkinkan terjadinya optimalisasi terapi ARV dan efektifitas asuhan keperawatan pada pasien HIV/AIDS dan juga peningkatan kualitas hidup pasien. AbstractThe purpose of this study was to identify the patient’s and relative’s perception on HIV/AIDS & stigma amongst the people. The design of this qualitative research is phenomenology approach. Thirteen informants (7 patients and 6 relatives of the patients) participated the study. The demographic profile of the informants were male and female with age from 18 years old to 40 years old for the patient and 45-55 for relatives. The findings identified some themes which are: (1) Lack of knowledge about HIV/AIDS on patient and relatives (2) The patients were not well informed on their condition (3) Misperception on HIV/AIDS and mode of spreading of HIV/AIDS (4) HIV/AIDS patients are responsibled and deserved for having infected HIV/ AIDS (5) The effort of the patient & their relatives to say nothing about their condition because they are afraid to be isolated from the community (6) The feeling of being isolated by the family member or a certain group of the people (7) Intimidation and physical Teror (8) The feeling of desperate and (9) Patient’s hopes. The result of these studies could be beneficial to minimize stigma by improving the knowledge of the people where it may optimize the early detection on the high risk people. It could also optimizes the treatment of ARV & the nursing care to the HIV/AIDS patient and their quality of life during AIDS stage.
