Abstract

Acquired Immunodeficiency Syndrome (AIDS) is a disease caused by infection with Human Immunodeficiency Virus (HIV). The pharmacology treatment for the infection was antiretroviral therapy. The problem from the use of antiretroviral drugs (ARV) is the emergence of unwanted drug reactions (adverse drug reaction). The purpose of this study was to identify and analyze the incidence of adverse drug reactions from the use of antiretroviral drugs in the treatment of patients with HIV / AIDS that occurred in RSUD Gunung Jati Cirebon. This study involves 122 patients. Data were obtained from medical records and patient interview form. The data were evaluated with a descriptive analysis of demographic profile of patients and the percentage of adverse drug reaction (ADR) The results showed an adverse drug reaction (ADR) on the use of antiretroviral drugs in the treatment of patients with HIV / AIDS in RSUD Gunung Jati Cirebon. ARV regimens are the most widely used is the combination of Zidovudine + Lamivudine + Nevirapine / Efavirenz (AZT + 3TC + EFV) as much as 31.2% (38 people). Duration of therapy ARV in Seroja Clinic RSUD Gunung Jati Cirebon most is> 2 years - 4 years 28.7% (35 people). Types of adverse drug reaction (ADR), which occurs in the use of antiretroviral drugs in the treatment of patients with HIV / AIDS in RSUD Gunung Jati Cirebon include: headache (22.1%), fatigue (6.8%), anemia (9.3%), itching (14.4%), nausea / vomiting (20.1%), diarrhea (7.2%), lipodystrophy (2.0%), rash (11.3%), skin discoloration (1.6%), neuropathy (1.6%) and sleep disorders (3.6%).

Abstract

Skin whitening products are one of the cosmetic products that contain active ingredients that can suppress or inhibit the formation of melanin or eliminate melanin that has been formed so as to give a whiter skin tone. Limitations of knowledge about various whitening cosmetic products make people do not know the negative effects that arise if not careful. Mercury and hydroquinone are some active substances that are often misused by illegal cosmetics manufacturers. But in fact the abuse of mercury and hydroquinone is still common in whitening products. This activity was carried out with the aim of providing knowledge and skills to the community, especially waiting room patients in the outpatient clinic at Gunung Jati Cirebon Hospital in terms of checking the registration number for cosmetics. This activity was carried out with counseling methods and question and answer about the material hazards of using whitening cosmetics containing mercury and Hydroquinone is followed by training in checking the registration number for cosmetics using an Android cellphone. From this activity it can be concluded that patients are more aware of the dangers of mercury and hydroquinone and can directly check cosmetic products that are commonly used everyday.

Abstract

Off-label adalah penggunaan obat atau peresepan obat dengan indikasi yang berbeda dengan yang tertera dalam label. Banyak laporan terkait penggunaan obat off label dalam terapi penyakit obstetri dan ginekologi, namun belum diketahui tingkat keamanannya untuk pasien. Penelitian ini bertujuan untuk mengetahui karakteristik pasien, karakteristik pengobatan, dan skrining penggunaan obat off-label pada pasien Obstetri dan Ginekologi di RSD Gunung Jati Kota Cirebon. Penelitian ini merupakan penelitian deskriptif observational dengan metodecross-sectional, pengumpulan data dilakukan secara retrospektif dan pengambilan sampel dengan cara Random Sampling berupa data rekam medik pasien Obstetri dan Ginekologi yang berobat pada bulan Juli-Desember 2018. Data dianalisis univariat untuk memperoleh frekuensi dan persentase penggunaan obat off label pada pasien Obstetri dan Ginekologi RSD Gunung Jati Cirebon. Dari 140 sampel rekam medik pasien Obstetri dan Ginekologi di RSD Gunung Jati Kota Cirebon diperoleh karakteristik pasien terbanyak berusia 23-28 tahun sebesar 33,57 % (47 orang), sedangkan untuk kisaran usia kehamilan yang paling banyak adalah berus ia 32-36minggu sebesar 47,14 % (66 orang). Karakteristik pengobatan yang paling banyak digunakan adalah obat nifedipin. Penggunaan obat off label sebesar 73,80% (200 orang) dimana obat off label terbanyak adalah nifedipin sebanyak 52,50% (105 orang) sebagai tokolitik, yaitu mencegah kontraksi uterus pada pasien Obstetri dan Ginekologi RSD Gunung Jati Cirebon.

Abstract

Respon terapi ARV ditunjukkan salahsatunya dengan perbaikan jumlah sel CD4. Kepatuhan pasien merupakan hal kritis untuk mendapatkan kemanfaatan dari terapi ARV. Tujuan penelitian ini mengetahui karakteristik pasien dan pengobatan, perubahan jumlah sel CD4, tingkat kepatuhan pasien dan hubungan tingkat kepatuhan penggunaan obat Anti Retroviral terhadap perubahan jumlah sel CD4 pada pasien HIV/AIDS di Klinik Seroja RSUD Gunung Jati Kota Cirebon. Penelitian ini merupakan penelitian observasional, melibatkan 75 pasien. Data diperoleh dari rekam medik dan hasil kuesioner Morisky Modified Scale (MMS). Hasil penelitian menunjukkan karakteristik pasien berdasarkan usia terbanyak kisaran usia 31-40 tahun 48 orang (64 %), jenis kelamin laki-laki 42 orang (56 %), tingkat pendidikan SMA 38 orang (50,7%), pekerjaan swasta 45 orang (60 %). Karakteristik pengobatan terbanyak menggunakan kombinasi Tenofovir + Lamivudin + Evafirenz sebanyak 34 orang (45.3%). Perubahan jumlah sel CD4 terjadi kenaikan pada 54 orang (72%) dan penurunan pada 21 orang (28%) selama 1 tahun terakhir. Tingkat kepatuhan pasien terbanyak yaitu tingkat kepatuhan tinggi sebanyak 33 orang (44%). Ada hubungan antara tingkat kepatuhan penggunaan obat Anti Retroviral (ARV) terhadap perubahan jumlah sel CD4 pada pasien HIV/AIDS di Klinik Seroja RSUD Gunung Jati Kota Cirebon, dilihat dari nilai p value 0,000 (p < 0,05).

Abstract

Drug interactions are one of part of Drug Related Problem (DRP) which significantly affect the development patient health. This study aims to determine the characteristics of kidney failure patients (age and genitals), an overview of the number of potential drug interactions, an overview of the types and amounts of other drugs interacting, and the severity of drug interactions in kidney failure patients in the Outpatient of “X” Hospital Cirebon City. This study involves 126 patient prescriptions. This type of research is a non-experimental research with descriptive analysis method, and using data collection methods with prospective random sampling technique in the Outpatient of RS “X” Cirebon. The results of the research from 126 samples obtained showed that the characteristics of Kidney Failure patients were predominantly male as many as 64 people (50.79%), the most age was in the age range 61-70 years as many as 38 people (30.16%). drug interactions 78 sheets of prescriptions (61.9%), the most severity of drug interactions was the strict / significant monitoring of 128 events (64.65%), the average of drug interactions per prescription sheet was 3 interactions, the type and number of the most interacting drug was furosemide + folic acid with 32 potential interactions (41.02%).

Abstract

The increase in the number of diarrhea patients every year at the Cirebon Lurah health center, of course, needs to be a special attention to this disease, where research is aimed at finding out the profile of the use of antidiarrhea drugs in toddlers in the Cirebon Lurah Health Center in the period January – December 2019. Using retrospective study design, using patient prescriptions to see the treatment patterns of toddler patients at Puskesmas Lurah Cirebon, patient characteristics were obtained, and the profile of antidiarrhea medication use during the period january to December 2019. From the results of the prescription study of the period obtained results and conclusions: The age that has the largest number of cases is the age of 0-12 months, which is as many as 74 cases (30.84%), The number of patients with diarrhea toddlers most gendered male 138 cases (57.5%), The most widely used types of antidiarrheal drugs are zinc 2,300 tablets, drug doses and rules of use of drugs in the treatment carried out to overcome diarrheal diseases in toddlers in puskesmas Lurah is in accordance with MTBS guidelines unless oralit is not in accordance with MTBS.

Abstract

IntisariLatar belakang: Acquired Immune Deficiency syndrome (AIDS) merupakan suatu sindrom atau kumpulan gejala penyakit dengan karakteristik defisiensi imun yang berat, yang merupakan manifestasi stadium akhir. Anti retroviral (ARV) adalah obat yang dapat menekan perkembangan HIV dalam tubuh. Beberapa ARV yang biasa digunakan di Indonesia antara lain Lamivudin dan Zidovudin. Kepatuhan pada pengobatan ARV telah diketahui sebagai komponen penting untuk mencapai keberhasilan suatu program terapi yang optimal. Tujuan: Penelitian ini bertujuan untuk mengetahui karakteristik pasien, karakteristik pengobatan, tingkat kepatuhan dan faktor-faktor yang mempengaruhi tingkat kepatuhan pasien HIV AIDS (karakteristik pasien dan jenis terapi ARV) di klinik Seroja RSUD Gunung Jati Kota Cirebon. Metode: Penelitian ini melibatkan 75 pasien, menggunakan kuesioner Morisky Modifikasi Scale (MMS). Hasil: Hasil yang diperoleh dilakukan analisis deskriptif untuk data karakteristik pasien, karakteristik pengobatan, tingkat kepatuhan pasien, dan dilakukan uji statistik Chi square untuk mengetahui korelasi faktor individu pasien terhadap tingkat kepatuhan. Hasil penelitian menunjukkan bahwa dari jenis kelamin yang terbanyak yaitu laki-laki sebesar 56%, usia yang terbanyak yaitu usia 31-40 tahun sebesar 64%, tingkat pendidikan yang terbanyak yaitu SMA sebesar 50,67%, dan pekerjaan yang terbanyak yaitu swasta sebesar 60%. Karakteristik pengobatan yang terbanyak yaitu kombinasi Tenofovir+Lamivudin+Efavirenz sebesar 45,3%. Tingkat kepatuhan pasien HIV/AIDS di Klinik Seroja RSUD Gunung Jati Kota Cirebon yang sedang 34,7% dan rendah 21,3%. Kesimpulan: Hasil Chi Square Tes menunjukkan bahwa tidak ada hubungan antara karakteristik pasien usia, jenis kelamin, tingkat pendidikan, pekerjaan dan jenis terapi ARV terhadap kepatuhan pasien HIV/AIDS.Kata kunci : HIV/AIDS, Anti Retroviral (ARV), Kepatuhan pasien Level of HIV/AIDS patient adherence to use of antiretroviral (ARV) drug in RSUD Gunung Jati CirebonAbstractBackground: Acquired Immune Deficiency Syndrome (AIDS) is a syndrome or symptoms with severe immune deficiency as a late-stage manifestation. Antiretroviral (ARV) is a drug that can suppress the development of HIV in the body. Some ARVs commonly used in Indonesia include Lamivudin and Zidovudin. Adherence to antiretroviral treatment has been recognized as an essential component for achieving an optimal therapeutic program.Objective: This study aimed to determine the characteristics of patients, treatment characteristics, adherence levels, and factors that affect the level of adherence of HIV/AIDS patients (patient characteristics and types of antiretroviral therapy) at Seroja Clinic RSUD Gunung Jati Cirebon. Method: The study involved 75 patients, using the Morisky Modified Scale (MMS) questionnaire. Result: The results obtained were descriptive analysis for patient characteristic data, treatment characteristics, patient adherence level, and Chi square statistical test to identify the correlation between patient's individual factors and adherence level. The results of the research showed that 56% of the patients were male, 64% of them aged 31-40 years, 50.67% went to SMA (high school) as their highest education attainment, and 60% took private jobs. The most treatment characteristic was the combination of Tenofovir + Lamivudin + Efavirenz at 45.3%. The adherence level of HIV/AIDS patients in Seroja Clinic of RSUD Gunung Jati Cirebon was currently 34.7% (moderate) and low (21.3%). Conclusion: The Chi Square Test results showed that there was no correlation between such patient characteristics as age, sex, education level, occupation, and types of antiretroviral therapy and HIV/AIDS patient adherence. Keywords: HIV/AIDS, Anti Retroviral (ARV), Patient adherence
