Good occupational health and safety (K3) needs to be done in an effort to reduce the possibility of accidents when doing work. To reduce the number of work accidents, it is necessary to control risk in the workplace through risk management, one of the methods is HIRARC. PT XYZ is a company engaged in providing environmental laboratory services. This company has several fields namely administration, analyst, test sample taker. Thearea most at risk is the part of the test sample taker. The purpose of this study is a Risk Assessment on the work of taking test samples at PT. XYZ. This type of research is qualitative with descriptive observational method, by describing the results of hazard identification and risk assessment on the work of taking test samples, in order to obtain a recommendation for hazard risk control using the HIRARC (Hazard Identification Risk Assessment Risk Control) method. Data collection techniques were carried out by observation, in-depth interviews and documentation. Sampling work carried out by test sampling officers PT. XYZ has 26 potential hazards. Test sample taker officer PT. XYZ has a low including the risk of skin irritation, eye irritation, minor injury, moderate includes the risk of muscle pain, fractures.and high includes the risk of injury to death, fire, heat stress, dehydration, respiratory infections. The recommended control refers to the control hierarchy, namely elimination, substitution, engineering, administrative and personal protective equipment. The conclusion of the test sampling job shows that the hazard identification stage of this work produces 26 potential hazards and has a risk level of low, moderate, high. The recommended risk control that has the most priority is the implementation of the OHS management system because the company is required to apply 64 basic criteria

Potential hazards in the workplace can result in work accidents and occupational diseases. One of the controls against work accidents is the availability of a first aid kit in the workplace. This study aims to evaluate the application of First Aid in Kalimas Port according to Permenakertrans No. 15 of 2008 concerning First Aid in the Workplace. This study used an observational study with a cross sectional. The population in this study were 53 Kalimas Port workers who worked in the warehouse area of PT. Port of Indonesia (Persero) Regional 3 Sub Regional Java. Samples were taken as many as 30 workers with accidental sampling technique. Collecting data using questionnaires and observation sheets based on Permenakertrans No. 15 of 2008. The results showed that 21 workers (70%) did not know the implementation of First Aid in Kalimas Port. The requirements for the first aid officer at Kalimas Port have been in accordance with the regulations set by the Minister of Manpower and Transmigration No. 15 of 2008 which is certified, physically and mentally healthy. The completeness of the first aid facilities, such as the appropriate room, of the 8 first aid boxes available, the contents of all the boxes did not meet the standards; the placement is not clearly visible, 6 of them are located in an inadequate room; 2 boxes are hard to reach; and there is no green first aid symbol yet. There is no means of transportation (ambulance) and shower in additional facilities. The application of First Aid at Kalimas Port was not appropriate, it was found that the box and contents of the First Aid were not in accordance with the standards and workers' knowledge of the application of First Aid was still lacking. Therefore, it is recommended for the company to carry out socialization to workers, improve maintenance and create a logbook for monitoring the first aid kit and contents, adding amap to find out the location of the first aid kit at the Kalimas port.

Lighting is one of the potential hazards and included in physical factors. If the quality of lighting in a room is not up to standard, it will be at risk to affect the comfort and safety of space users. This study aims to analyze the relationship between lighting systems and eye fatigue on employees in the ABC Division at the XYZ Office of Gresik Regency. This study is an analytic observational study with a cross-sectional. The sample in this study were 13 respondents using a total sampling technique. Measurement of lighting intensity was done using a lux meter and eye fatigue was measured using a Visual Fatigue Index. To see the strength of the relationship between variables, the Contingency Coefficient. The results showed 11 points (84.6%) with lighting intensity that not up to standards and 11 employees (84.6%) experienced eye fatigue. The lighting system has the strongest relationship with eye fatigue (P Value =0.707). Recommendations that can be given are to check the cleanliness and function of the armatures and lamps regularly, change the color of wall paint regularly, install curtains on windows, fill the room with green ornamental plants, socializing and implementing about 5R culture and eye stretching.

Although it has a good impact on work efficiency and effectiveness, the use of computers in office workers also has aspects that can pose an ergonomic risk, namely musculoskeletal disorders (MSDs). To describe the complaints of MSDs and the level of ergonomic risk in PT. XYZ employees can use the NBM and ROSA methods. This research is a quantitative descriptive with an observational and cross sectional approach. The population of this research is employees of the head office of PT. XYZ working on a computer. The sample collection technique used is stratified random sampling according to the inclusion and exclusion criteria and data collection is done by filling out the Nordic Body Maps (NBM) questionnaire, direct observation and documentation. The results of the study based on the distribution of MSDs complaints and their percentages showed that the level of complaints often experienced by workers was on the left shoulder, buttocks, right forearm and fingers of the right hand. Furthermore, the measurement of the level of ergonomic risk using the ROSA method got the results of 52 people including the category of moderate risk level and 4 people belonging to the category of high risk level. In conclusion, the description of subjective complaints and the level of ergonomics risk on respondents needs to be considered and further corrective actions such as improvement of work stations and work postures of employees and socialization of office ergonomics to reduce the level of ergonomics risk using posters.

Poor work posture and a low level of work skills can result in injury to the work hazard of Musculosceletal Disorders (MSDs). Work postures that are not ergonomic are at risk for experiencing complaints of Musculosceletal Disorders (MSDs). One of the workers who are at risk for experiencing Musculosceletal Disorders complaints are workers who still carry out manual lifting and transporting jobs, such as the wholesale varnish workers at PT. SPINDO Tbk, Unit II. The purpose of this study was to describe work postures with complaints of Musculosceletal Disorders in lacquer wholesale workers at PT. SPINDO Tbk, Unit II. This research method is quantitative descriptive with a cross sectional study approach. The number of samples is the same as the population, which is 9 people taken using the total population. Data collection consists of primary data such as the number of workers, age, length of service, years of service, education level obtained from the company. Primary data such as work posture using the REBA assessment sheet while complaints of Musculosceletal Disorders using the Nordic Body Map assessment sheet. The results of this study indicate that most of the lacquer wholesalers work with a working posture with a moderate risk category of 66.6%, so that the complaints of Musculosceletal Disorders felt by workers in the slightly sick category are 28.5% of the right upper arm. Therefore, it is recommended for companies to provide education to workers regarding ergonomic risk control techniques in terms of work attitudes to reduce complaints of Musculosceletal Disorders while workers should stretch before starting work.
