Abstract

ABSTRAKLatar Belakang.Limfoma malignum sel B jenis sel besar difus adalah Limfoma malignum non-Hodgkinyang sering dijumpai. Penyakit ini memiliki gambaran klinis, morfologik, perubahangenetik, dan molekular yang heterogen. Sel limfoma dapat memiliki fenotip serupadengan sel sentrum germinativum normal ditandai dengan ekspresi CD10 dan Bcl6 yangdapat berhubungan dengan gejala klinis dan prognosis yang baik. Penelitian ini inginmenilai hubungan imunoekspresi CD10 dan Bcl6 dengan skor Indeks PrognostikInternasional (IPI).CaraPenelitian dilakukan secara retrospektif pada 22 sediaan kasus Limfoma malignum sel Bjenis sel besar difus dari Januari 2007 sampai Juni 2009 di Bagian Patologi AnatomikRumah Sakit Dr. Soetomo Surabaya. Semua bahan dilakukan penilaian ulang untukmenentukan diagnosis berdasarkan klasifikasi WHO 2001 berupa pulasan HE danimunohistokimia dengan CD10 dan Bcl6. Uji korelasi kontigensi digunakan untuk analisadata.HasilHasil penelitian ini menunjukkan hubungan yang bermakna antara ekspresi CD10 danekspresi Bcl6 dengan skor IPI (p< 0,05). Tidak terdapat hubungan bermakna antaraekspresi CD10 dengan Bcl6 (p > 0,05).KesimpulanEkspresi CD10 dan Bcl6 berkaitan dengan prediksi prognosis.

Abstract

Intravascular lymphoma is a rare type of lymphoma with some fundamental differences compared to other types of non–Hodgkin lymphoma in general. Its clinical manifestations vary greatly and depend on the affected organs. There is no pathognomonic signs of intravascular lymphoma, either in clinical, laboratory or radiological findings. This case is challenging to detect and often misdiagnosed because it does not show symptoms that are commonly found in other types of lymphoma, such as mass or nodules formation. The treatment is relatively similar to other lymphoma in general, but the prognosis is worse because it was mostly diagnosed in the advanced stage. These specific conditions become the main reason to separate this case as a special entity of lymphoma.

Abstract

Henoch-Schönlein purpura (HSP) is an acute immunoglobulin A (IgA)–mediated vasculitis involving the small vessels. This disease cause systemic involvement of many organ especially the skin, the gastrointestinal (GI) tract and the kidneys. The etiology of HSP is not clear. Precipitating factors include drugs, chemicals, viruses and bacteria. Pathogenesis of HSP involve Ig A–mediated immune complex that are circulated in blood vessel and deposited in many organs. The presence of IgA-mediated immune complex deposits will activate the complement system, causing inflammation in the form of vasculitis that damage the small blood vessels in many organs. Hsp is a self-limiting disease that require supportive therapy. The problem that arises is how to distinguish HSP with other vasculitis diseases. A skin biopsy in patients with hsp will reveal leukocytoclastic vasculitis in small vessel. That kind of vasculitis are also found in urticarial vasculitis, hypersensitivity vasculitis and vasculitis due to cryoglobulinemia. Direct Immunofluorescence test from skin biopsy tissue will help to make the diagnosis of HSP. The presence of IgA deposits in small blood vessels wall can distinguish HSP from other vasculitis.

Abstract

Introduction: Data from Indonesian Health Ministry stated that there was an increase in the prevalence of breast tumors in 2015 – 2017 by 1.8 per 100 to 21.3 per 100,000 women aged 30-50 years. The most common types of benign breast lesions found in women worldwide are fibrocystic changes of breast and fibroadenoma. Aim: This research aims to analyze the factors of age, age at menarche, parity, use of hormonal drugs, and family history with the incidence of benign breast lesions in RSUD Dr. Soetomo Surabaya. Methods: This research method is observational analytic with a case-control approach, consisting of a case group and a control group of 41 respondents in each group. The statistical test of the study used the Chi-Square Test, Independent t-test, Mann Whitney, and Odds Ratio (OR). Results: The results of statistical tests obtained age factor p(sig)=0.002, hormonal use p(sig) 0.000, family history p(sig) 0.016, age at menarche p(sig) 0,282, and parity p(sig) 0.568. Conclusion: Conclusion there is a relationship between age, use of hormonal drugs, and family history and there is no relationship between age at menarche and parity with the incidence of benign breast lesions in RSUD Dr. Soetomo Surabaya. Abstract

Colorectal adenocarcinoma is one of the most prevalent malignancies worldwide. The presence of regional lymph node metastases is an important prognostic indicator in colorectal adenocarcinoma. CD44 is a transmembrane protein and a cancer stem cell biomarker that plays a role in tumor proliferation, metastasis, and recurrence. The tumor microenvironment has a vital role in the stage of tumor development. CD8 is one of microenvironment's component which kills the tumor cells. This study aimed to analyze the expression of CD44 and CD8 and investigated their association with colorectal adenocarcinoma metastatic. This study's benefit is that the resulting study can be used as a prognostic factor in colorectal adenocarcinoma. This study included 56 paraffin blocks of patients diagnosed as colorectal adenocarcinoma during January 2017-December 2018 in Anatomic Pathology Laboratory Dr. Soetomo Hospital Surabaya. The paraffin blocks were sliced and stained with immunohistochemistry for CD44 and CD8. The parameters were a positive percentage of tumor cell and lymphocyte immunoreactivity against CD44 and CD8 antibodies. The percentage of CD44 is higher in the metastatic group, and the percentage of CD8 is higher in the non-metastatic group. There was a significant negative correlation between CD44 and CD8 expression in regional lymph nodes metastatic. These results indicated that CD44 and CD8 have a vital role in the incidence of metastasis.

Abstract

Breast cancer is the most common cancer in women throughout the world, with new cases and deaths which continue to increase. Soursop leaves (Annona muricata L) have been used extensively in traditional medicine, including cancer. Acetogenin, alkaloids, and phenols contained in soursop leaves are known to have anti-cancer effects. Among them, acetogenin has the most dominant role and reported to have a cytotoxic effect on various cancer cell lines. This study aims to determine the cytotoxic activity of soursop leaf ethanol extract on T47D breast cancer cell line. Measurement of cytotoxic activity was carried out by the MTT method, and the viability percentage of T47D cells was calculated based on the absorbance values in the treatment, cell control, and media control groups of each replicate. The correlation between extract concentration and viability percentage of the T47D cell line was outlined in the regression equation to obtain the IC50 value. IC50 values of 109.91 ± 3.04 with R values 0.975 and R2 0.9508 obtained. R values close to 1 indicated a strong correlation between extract concentration and the percentage of living T47D cells. Meanwhile, the amount of R2 suggested that the level of AMEE had a 95.08% influence on the rate of cell viability, and the other 4.92% influenced by factors other than the AMEE dose. These results indicated that the ethanol extract of soursop leaves has a cytotoxic effect and has the potential to inhibit T47D cell proliferation in vitro.

Abstract

Abstract Goat bile has been used by some Indonesian people to treat malaria and increase their stamina. This study aimed to prove whether goat bile toxic or not in BALB/c mice and to verify the antimalarial activity of goat bile at various concentrations in mice infected with Plasmodium berghei ANKA. Acute toxicity test was performed using twenty male BALB/c mice with an average body weight of 25 grams, which were divided into four groups. Mice were given 25%, 50%, and 100% goat bile, respectively, while negative control was given distilled water. Any change in weight, odor, color, agitation, appearance, color of urine and feces, coma, and death, were recorded. A different set of mice were infected with P. berghei ANKA. This study conducted using the posttest only control group design with four treatments and five replications. A four day-treatment of goat bile was given by oral gavage to find out its effect on parasitemia level. Infected mice were divided randomly into 4 groups, where the GBNeg group as negative control was given only distilled water. The GB25, GB50, and GB100 groups were treated with 25%, 50%, and 100% goat bile, respectively. The parasitemia was observed daily on Giemsa-stained tail blood smears of each mice. No death or other sign of toxicity was found in goat bile-treated mice. Goat bile showed anti-malarial activity. The parasitemia in all goat bile treated groups was lower compared with the negative control group. The ED50 of goat bile against the growth of parasite was 48,55 %. Goat bile is a potential source of new antimalarial therapies. Abstract

Abstract Sequestration of Plasmodium berghei ANKA-infected erythrocytes occurs in BALB/c mice as characteristic of  Plasmodium falciparum infection in humans. Animals’ bile has been widely used for centuries in Traditional Chinese Medicine. Goat bile has been used in healing infectious and non-infectious diseases; however, no report on the use of goat bile against malaria infection and sequestration. The purpose of this study was to analyze the correlation between parasitemia and sequestration in the liver of P.berghei ANKA-infected BALB/c mice treated with goat bile. This research was an in vivo experimental study using the post-test control group design. The male BALB/c mice aged ± 6 weeks, body weight 20-25 g were used. The mice were divided into five groups where Group 1-3 were mice treated with goat bile 25%, 50%, and 100%, respectively. Group 4-5 were negative (sterile water) and positive controls (DHP). Parasitemia was observed daily from each mouse and the number of sequestered infected erythrocytes on the endothelium of sinusoids. The data were analyzed using t independent test. Antimalarial activity of goat bile was shown by the lower parasitemia in goat bile-treated mice compared with the negative control. The average number of sequestration was goat bile concentration-dependent manner. The higher the concentration, the lower the number of sequestration. Sequestration was correlated with parasitemia (p=0,0001). Abstract

Sepsis adalah suatu sindroma klinik yang terjadi karena adanya respon tubuh yang berlebihan terhadap rangsangan produk mikroorganisme. Pasien sepsis dengan infeksi bakteri penghasil ESBL 57,4% terinfeksi E. coli, 21,35% terinfeksi Enterobacter sp, dan 21,3% terinfeksi Klebsiella sp. Sepsis dapat diperberat oleh peningkatan kuman yang multiresisten terhadap bermacam antibiotik, seperti E. coli ESBL dan K. pneumoniae carbapenemase (KPC). Selama infeksi bakteri, faktor virulen diproduksi dan disekresikan dari patogen dan memicu sinyal apoptosis. Penelitian mengenai caspase 3 dianggap penting, karena caspase 3 merupakan efektor caspase terpenting yang bertanggungjawab atas morfologi dan perubahan biologis yang terlihat pada sel apoptosis. Jenis penelitian ini adalah true experimental dengan rancangan penelitian post test only control group design. Hasil penelitian menunjukkan bahwa ekspresi caspase 3 pada limpa mencit yang diinfeksi KPC adalah 65,25±12,69%, sedangkan yang diinfeksi E. coli ESBL adalah 33,75±3,862%, hal tersebut diduga karena adanya kapsul polisakarida yang mengelilingi KPC dan melindungi dirinya terhadap aksi fagositosis dan bakterisidal serum yang dapat dianggap sebagai penentu virulensi paling penting dari KPC dan menyebabkan mitokondria melepaskan ROS. Infeksi bakteri tersebut menyebabkan mitokondria memproduksi ROS dan memicu pelepasan sitokrom c. Sitokrom c akan memicu caspase 9 untuk berikatan dengan efektor caspase 3, sehingga terjadi apoptosis Kata kunci: Sepsis, ekspresi caspase 3, limpa tikus, Escherichia coli ESBL, Klebsiella pneumoniae carbapenemase

Abstract

The protein energy deficiency cause intestinal villus atrophy and epithel mucous damage. The effect of bee honey on histostructure of intestine was studied in the experimental mice as model of proteinenergy deficiency. The use bee honey in protein-energy deficiency shown to improve intestinal villus atrophy and epithel damage. In conclusion that bee honey can use as alternative medicine in protein energydeficiency
