Abstract

Background: Snakehead fish has been associated with wound healing due to its high albumin content. Albumin can accelerate the inflammatory process so that tissue repair will be faster. Lack of albumin protein causes a person tend to experience prolonged wound healing. Purpose: This study aims to prove the effect of snakehead fish extract on the number of macrophages and blood vessels on the inflammatory reaction of skin wound tissue in rats. Methods: Laboratory experimental design with a total sample size of 27 male rats have undergone incisions, divided into 3 random groups: control group was given aquadest, treatment group 1was given aquadest and 50% extract of snakehead fish, and treatment group 2 was given 100% extract of snakehead fish. The rat’s skin wound tissues were taken on day 5 and observed under the microscope. The average number of macrophages and blood vessels on granulation tissue that formed at the base of the wound were counted and then performed data analysis. Result: In this study, we found a down wards trend in the average number of macrophages and blood vessels in treatment group 1 and treatment group 2. The control group and the treatment group 2 showed a significant difference, however, between the other groups did not show significant difference. Conclusion: There is a significant effect of giving 100% snakehead fish extract on decreased macrophages and blood vessels in the inflammatory reaction of skin wound tissue.

Abstract

Highlight:Profile of skin biopsy results which carried out the anatomical pathology examination was reviewed.The highest skin biopsy disease group case are erythropapulosquamous, infection, skin tumor, vesiculobullous, connective tissue disease, pigmentation disorders, and vasculitis Abstract:Skin biopsy is an important tool used by dermatologists in diagnostic determination. The correlation between clinical and histological features is needed in understanding pathogenesis and formulating the diagnosis of a skin disease with a greatly varied spectrum of histopathological results, while the observable clinical symptoms are highly limited. Skin diseases are still a serious problem worldwide, especially in Indonesia. Based on the Indonesian Health Profile in 2010, skin diseases ranked third out of 10 most diseases in outpatients in hospitals throughout Indonesia. This study was a review of the profile of skin biopsy results in Dr. Soetomo General Academic Hospital, Surabaya, Indonesia from 1 July 2014 to 31 July 2019, which were subjected to anatomic pathology examination. This study was an observational descriptive study using secondary data sources from the medical records at the Communication and Information Technology Installation (ICT) of Dr. Soetomo General Academic Hospital, Surabaya. Based on data searches, the total number of biopsy examinations performed was 1,368 cases. There were more female patients (50.3%) than males (49.7%). The most common skin disorder found was erythropapulosquamous disorder (30%), followed by infection (18%). Other cases consisted of skin tumor (15%), vesiculobullous (13%), connective tissue disease (7%), pigmentation disorders (5%), and vasculitis (5%). Diseases that could not be classified into 7 groups of the biopsy criteria were grouped separately in other diseases (7%).

Abstract

Tumor and its microenvironment can interact each other. Macrophage is part of tumor microenvironment. New drugs targeting specific superficial receptor of macrophage or cytokine of macrophage polarization have been found. Therefore, macrophage phenotype and its ratio of M1/M2 macrophage need to be identified. This identification could lead us to prognose breast cancer and monitor its therapy. Analytical observational study with cross sectional approach, conducted on paraffin block sample of breast carcinoma from Anatomical Pathology Laboratory of Dr. Soetomo General Hospital. The samples divided into four groups based on nodal metastasis staging (N0, N1, N2, and N3) and stained with antibody against CD68 and CD163. The ratio of  CD68 and CD163 were analyzed with Anova test. There were difference expression of CD68 as M1 macrophage marker in various axillary node metastasis groups (p=0.015). There were difference ratio of CD68 as M1 macrophage marker and CD163 as M2 macrophage marker in various axillary node metastasis groups (p=0.005). There were difference ratio of macrophage M1 and macrophage M2 between N0 and N3 group, N2 and N3 group also.

Abstract

Scabies is a skin infection caused by Sarcoptes scabiei var. hominis. This disease may present severe clinical manifestations in immune-compromised patient, well-known as Norwegian scabies or crusted scabies.A 36-year old man with AIDS had chief complaint thick crust almost all over his body in this case. History of household member infected by scabies before was present. Clinical findings show hyperpigmented macules unsharply marginated, covered with thick scales and accompanied by papules, fissures, and erotion. T cell CD4 level was 12 cell/μL. Scraping examination showed scabies infection and so did the histopathology examination. This patient was treated by topical Permethrin 5% combined with 2-4 ointment application in between permethrin usage. Before topical scabicide was given, thick crust was previously treated by topical urea 10% and wet dressing by normal saline. On day 14 after the patient first came there was lesion improvement.

Abstract

Helicobacter pylori is a bacteria that commonly cause chronic gastritis. Identification of its infection is essential for eradication treatment. Detection of H.pylori bacteria in gastric biopsy specimen by histology method is a diagnostic tool that widely accepted because it is superior to serology examination. Although the bacteria can be seen in routinely Hematoxylin-Eosin staining, Modified Giemsa and Whartin Starry stain was commonly used to see the bacteria more clearly. Whartin Starry stain gives more contrast to the bacteria but modified Giemsa stain is preferable at many centres because it is a cheaper and simpler method. This study want to find out whether there is difference result in detection of H.pylori using these two stains. Material and methods. Paraffin blocks from gastric biopsy patients with chronic gastritis were retrieved from Anatomic Pathology Laboratory Dr.Soetomo Hospital Surabaya in the year 2017. Thirty paraffin blocks were taken randomly and were made into microscopic slides for staining with Warthin-starry and modified Giemsa stain concomitantly. Results. Specimen with Whartin starry stain show 19 out of 30 were positive for H.pylori while modified-Giemsa stain found 16 out of 30 specimen were positive for H.pylori. Whartin Starry stain use silver reagent to coat the bacteria so it become more clearly visible. Conclusion. Detection of H.pylori Warthin-starry stain give more chance to obtain positive result because it use silver technique that coat the bacteria making it is more clearly visible in microscopic examination.

Abstract

Background: Infection in wound could lead to various consequences including scarring and occurrence of sepsis. With increasing prevalence of resistant strain bacteria, option for treating infected wound need to be constantly expanded. Propolis, a product of honey bee, is a potential treatment which has been studied to have various effects including antibacterial, anti-inflammatory, and antioxidant. Objective: The study aims to assess the effect of propolis toward wound healing, by evaluating wound macroscopic and microscopic appearance. Material and method: Propolis extract were made using maceration technique using ethanol. Thirty Rattus norvegicus male rats were divided into six groups: three groups for treatment and three groups for control. Incision wound were made in the back and was infected with Methicillin ResistantStaphylococcus aureus (MRSA). Propolis was administered orally on daily basis. Wound excision for observation were done on day3, 5, and 7. The tissue were processed using haematoxylin-eosin (HE) staining for microscopic observation. The distance of normal tissue surrounding the wound were measured using image analysis program. Result: Macroscopic observation showed fewer visible wound in the propolis extract treated group. Measurement by microscopic observation on day 3 also demonstrated more narrow wound width in propolis extract treated group (405.75  110.11 μ) compared to the control group (806.20 644.41μ). Conclusion: The result of this study suggests that propolis extract by oral administration promotes the healing of infected wound as shown within microscopic and microscopic observation.

Abstract

Ovarian cancer is the fourth cancer with most incidence in Indonesian female with 10.238 cases in 20141. Tumor marker CA-125 is assosciated with ovarian cancer, importantly epithelial ovarian cancer. This study aims to find out diagnostic value (sensitivity, specificity, positive predictive value, negative predictive value) of CA-125 among patients with epithelial ovarian cancer in Dr. Soetomo General Hospital Surabaya in 2016. This study used analytic cross sectional method and was performed by evaluating medical records of patients suspected for ovarian malignancy in Dr. Soetomo General Hospital Surabaya in 2016. There were total 97 patients found fit for criteria of inclusion in this study. Tissue histopathological examination confirmed 66 patients have epithelial ovarian malignancy and 31 patients do not. Samples distributed using 35 U/ml as CA-125 upper limit, TP: 54.64%, FP: 19.59%, FN: 13.40%, dan TN: 12.37%. Diagnostic value obtained as follows: sensitivity 80.30%, spesificity 38.71%, positive predictive value 73.61%, negative predictive value 48%, and accuracy 67.01%. Tumor marker associated with ovarian cancer CA-125 has found high in sensitivity but low in specificity among patients with epithelial ovarian cancer in Dr. Soetomo General Hospital Surabaya in 2016.

Abstract

Kanker payudara adalah tumor ganas yang terbentuk dari sel-sel payudara yang tumbuh dan berkembang tanpa terkendali sehingga dapat menyebar ke jaringan atau organ yang berada dekat dengan payudara atau ke bagian tubuh lainnya. Annona muricata Linn atau Graviola telah digunakan sebagai obat tradisional untuk menangani berbagai jenis penyakit seperti demam, rematik, kanker dan juga sebagai sedatif, insektisida dan imunosupresif. Pada penelitian ini, cell line kanker payudara MCF-7 diberi ekstrak etanol A. muricata kemudian diinkubasi 24 jam. Setelah itu dilakukan uji sitotoksik menggunakan metode MTT assay. Parameter yang digunakan pada uji sitotoksik adalah IC50 yang menunjukkan konsentrasi inhibisi sel kanker sebanyak 50% dengan nilai IC50 sebesar 117,87 μg/ml. Ekstrak dapat dikatakan memiliki aktivitas sitotoksik jika nilai IC50 kurang dari 1000 μg/ml setelah 24 jam kontak dengan sel kanker. Dengan demikian, dapat disimpulkan bahwa ekstrak etanol daun A. muricata dapat digunakan sebagai pilihan terapi anti-kanker alami untuk kanker payudara.Kata Kunci:  Kanker payudara, Ekstrak etanol daun Annona muricata, MCF-7, MTT assay

Abstract

Perubahan klinis pada payudara sebagian besar bersifat jinak, hanya 3 – 6% yang merupakan keganasan payudara. Namun demikian, kanker payudara menjadi perhatian utama mengingat angka kejadian yang terus menerus meningkat serta morbiditas dan mortalitas yang tinggi. Pengobatan kanker hingga saat ini masih menjadi tantangan. Hal ini membuka peluang penelitian terapi kanker terutama dari bahan herbal. Daun sirsak telah banyak digunakan masyarakat secara tradisional untuk pengobatan tumor. Di samping itu berbagai studi juga telah menunjukkan efek sitotoksik tanaman ini terhadap berbagi cell line kanker. Namun demikian belum ada penelitian yang menganalisis efek ekstrak daun sirsak terhadap populasi limfosit di lingkungan mikro tumor terutama yang berfungsi pada respon imun anti tumor. Penelitian ini bertujuan untuk mengetahui infiltrasi sel limfosit T CD8 pada jaringan tumor yang payudara tikus Sparaque Dawley yang diberikan ekstrak etanol daun sirsak. Ekspresi CD8 dianalisis melalui pewarnaan imunohistokimia. Ekspresi CD8 positif ditandai dengan warna coklat pada sitoplasma sel limfosit, selanjutnya ekspresi CD8 dari setiap sampel dihitung pada 3 lapang pandang dengan high-power field, kemudian ditentukan nilai rata-ratanya. Pewarnaan HE menunjukkan variasi infiltrasi limfosit pada nodul payudara tikus SD. Rata-rata ekspresi CD8 kelompok kontrol menunjukkan hasil lebih tinggi dibandingkan kelompok perlakuan. Namun demikian, hasil uji komparasi menunjukkan tidak ada perbedaan bermakna  (p > 0,05) antara ekspresi CD8 kelompok kontrol dengan perlakuan. Hasil ini menunjukkan bahwa pemberian ekstrak daun sirsak tidak menyebabkan perbedaan bermakna ekspresi CD8 pada populasi limfosit yang menginfiltrasi tumor payudara tikus SD.

Abstract

ABSTRACTIntroduction: Dyslipidemia is a lipid metabolism abnormality marked by increase or decrease of lipid fraction in the plasma. Statin or 3-Hydroxy-3-Methylglutaryl coenzyme A (HMG-CoA) reductase inhibitor is an important dyslipidemia medication that may induce apoptosis of the skeletal muscle.Decrease of Bcl-2 and Bax ratio causing caspase-9 activation followed by caspase-3 activation occur in apoptosis. Moderate intensity physical exercise (MIPE) decreases DNA fragmentation, increases Bcl-2 protein level and decreases Bax transcript level.Methods: Thirty healthy BALB/C strain Mus musculus divided into 3 groups: first intervention group with statin only treatment (S), second intervention group with statin treatment plus MIPE using motorized treadmill (ST), and control (C) group with no statin and nor exercise. The duration of treatmentwas 28 days. Statin treatment dose was 2.06mg/kg bodyweight. Measurement caspase-3 was done by immunohistochemistry evaluation and the expression of capase-3 was calculated by cells count which expressing caspase-3.Results: There were more muscle cells expressing caspase-3 in the S group (47.00±12.92cells) than the C group (2.70±1.94 cells); p=0.000, the ST group (21.44±10.13 cells) than the C group (p=0.001), as well as S group and the ST group (p=0.000).Conclusion: Moderate intensity physical exercise can decrease caspase-3 expression in statin treated subject.Keywords: apoptosis, caspase-3, dyslipidemia, moderate intensity physical exercise, statin.
