Abstract

Tuberculosis (TB) is still a threat to the community in Teluk Paman Village, Kampar Kiri District, Kampar Regency. In addition, it is not yet optimal for community empowerment by the government and health workers. This is indicated by the absence of a "Pos TB Desa" and TB cadres who can assist community-based TB management in the village. The strategy of empowering the community can be done in various ways, including advocacy, community development and community movements. The purpose of this community service was to examine the implementation of community empowerment in TB prevention and control and to see the effectiveness of advocacy and training in realizing empowered cadres. The empowerment method was advocacy, forming a "Pos TB Desa" and village TB control network, training with question and answer lecture methods combined with simulations and using various educational media. This activity has provided many benefits. The community has been able to utilize the TB Village Post that is already available. This activity helps the Health Office and local government in preventing and controlling TB cases. This activity also increases the knowledge and skills of village TB Post cadres to help prevent and control TB.

Abstract

Introduction: Chronic obstructive pulmonary disease (COPD) is often associated with cardiovascular disease because it has the same risk factors as smoking. One of the cardiovascular complications in COPD is heart failure. Echocardiography examination is a tool to evaluate changes of the heart both anatomically and functionally. This study aimed to determine the relationship between COPD and heart failure by echocardiography examination in stable COPD patients. Methods: This was a descriptive-analytic study using a cross-sectional design for COPD patients who visited the Lung Polyclinic of Arifin Achmad Hospital, Pekanbaru, Riau, which was conducted from November 2020 to March 2021. Subjects who met the inclusion criteria were examined by spirometry, chest X-rays, and echocardiography. Results: In this study, there were 66 stable COPD subjects. It was found that 15.2% of subjects had right heart failure based on the tricuspid annular plain systolic excursion (TAPSE) assessment and 7.6% of the subjects had left heart failure based on the left ventricular ejection fraction (LVEF) value. Right ventricular dilatation (p = 0.000), right ventricular dysfunction (p = 0.005), and increased probability of pulmonary hypertension (PH) (p = 0.000) were significantly associated with a decrease in the first second forced expiratory volume (FEV1).Conclusion: The severity of COPD with FEV1 <50% had a significant relationship with dilatation, dysfunction of right ventricular, and increased the probability of incidence of PH based on parameters tricuspid regurgitation (TR). A history of exacerbations ≥2 times in a year caused right ventricular dilatation, decreased right ventricular function, and increased the probability of PH.

Abstract

Pulmonary tuberculosis is an infectious disease caused by Mycobacterium tuberculosis which mostly attacks the lungs and can also attack other organs. Tuberculosis is transmitted by smear positive tuberculosis patients, Tuberculosis germs are transmitted through the air when the patient coughs and sneezes so that the germs spread in the air in the form of droplets (sputum sparks), so people can become infected if they inhale the droplet into the respiratory tract (Ministry of Health Republic of Indonesia, 2014 ). According to Achmadi (2010) risk factors for tuberculosis transmission are environmental and behavioral factors. Environmental factors include ventilation, occupancy density, temperature, lighting and humidity. Whereas behavioral factors include smoking, spitting or sputum in any place, coughing or sneezing not closing the mouth and habit of not opening the window. Based on the results of a survey in Indonesia by the Directorate General of Communicable Disease Eradication and Environmental Health (2011), the high incidence of pulmonary TB is caused by a lack of knowledge. Knowledge of tuberculosis sufferers will affect the attitude and behavior such as throwing up spit or sputum that can be carelessly a source of transmission for people around him. Prabu in Fitriani (2013) explained that family heads who have an income below the regional minimum wage (UMR) will consume foods with nutrient levels that are not in accordance with the needs of each family member so that they have an nutritional status that is lacking and will make it easier to contract infectious diseases including Lung TB. According to WHO the number of cases of tuberculosis is ranked above HIV / AIDS. In 2016 there were an estimated 10.4 million new cases of tuberculosis or 142 cases / 100,000 population, 60% of new cases occurred in 6 (six) countries namely India, Indonesia, China, Nigeria, Pakistan and South Africa, and Indonesia ranked second (WHO Global Tuberculosis Report, 2017). Based on data on Indonesia's health profile in 2018 the number of tuberculosis cases in Indonesia in 2017 was 420,994 cases and increased in 2018 to reach 511,873 cases (Ministry of Health Republic of Indonesia, 2018). The number of pulmonary tuberculosis in Riau Province in 2018 is 11,135 cases (Ministry of Health of the Republic of Indonesia, 2018). Based on data on the health profile of Kampar Regency in 2017, tuberculosis is an environmentally-based direct infectious disease that ranks second with a total of 1,071 cases after diarrheal disease with a total of 1,076 cases. The number of positive AFB cases in 2017 in Kampar District reached 1071 cases, with the highest number of sufferers found in Tambang District, 78 cases. In 2018 the number of TB cases was 1079 cases, with the highest cases also occurring at the Tambang Health Center with a total of 67 cases and 43 new cases, followed by the Siak Hulu I Health Center with a total of 57 cases (Dinkes District Health Office, 2018). Based on this, this study aims to analyze and the risk of the relationship between the physical, social and economic environment with the incidence of pulmonary tuberculosis in Tambang District, Kampar Regency.

Abstract

This study aims to analyze the influence of physical environmental factors (clean water facilities, latrine conditions, SPAL conditions, type of house floor, household waste management), economic factors (family income) and social environmental factors (nail hygiene, use of footwear, washing habits hands with soap, bowel habits) against helminthiasis in elementary school students in Seberang Tembilahan Village, Indragiri Hilir Regency. The research location was conducted in Seberang Tembilahan Sub-District, Indragiri Hilir Regency in January to February 2020. The study used a survey method. The analytical method used is univariate analysis, bivariate analysis with Chi Square (Fisher) and multivariate analysis with logistic regression. The results of the study of 72 elementary school students in 4 elementary schools who were examined by laboratory feces, found positive results of worm infestation of 23.61% or 17 students. The final results of the model test showed that although the clean water facilities and toilet conditions were not significant, the two variables influenced each other, it was possible that there was an interaction between the two variables in influencing the incidence of helminthiasis in elementary school students. The variable that most affected the incidence of helminthiasis in elementary school students was the latrine condition with adjusted OR of 4.01 so that respondents whose conditions did not meet the requirements were 4.01 times more likely to experience helminthiasis compared to conditions that were eligible after being controlled by other variables. Simultaneously the two variables (clean water facilities and latrine conditions) can explain the variation of helminthiasis in elementary students by 9.9%.

Abstract

This study aims to (1) analyze the factors of physical environmental conditions and sanitation in realizing a healthy canteen (2) analyze the behavior of food handlers in realizing a healthy canteen on the Riau University campus (3) analyze policies related to efforts to create a healthy canteen on the Riau University campus (4) analyze the formulation of strategies and recommendations to realize a healthy canteen on the Riau University campus. This research was conducted in 8 canteens at the Riau University Campus, Pekanbaru City, which are located at the Bina Widya Campus, Gobah Campus and Diponegoro FK Campus. The tools and materials in this study used in this study were the meter, luxmeter, questionnaire, interview sheet which was used to measure the variables studied, by filling out the question sheet. This study uses a quantitative descriptive design approach, to obtain factors of environmental conditions and sanitation as well as food handler behavior. Data collection techniques used in this study were questionnaires, interviews and observations. Data on factors of physical environmental conditions were taken by observation, including canteen sanitation facilities and canteen equipment sanitation. Data on the hygiene factors of food handlers were taken by means of questionnaires and observations. The data taken include personal hygiene of food handlers, nail hygiene, use of aprons and shoes when cooking and hand washing habits. All data will be compared with the Regulation of the Minister of Health of the Republic of Indonesia Number 1096/MENKES/PER/VI/2011 concerning Food and Service Sanitation Hygiene. The entire data that has been collected is tabulated in a table and analyzed descriptively using Microsoft Excel software.Based on the research data, it is known that from the sanitation indicators assessed, some canteens still cannot meet the requirements to realize a healthy canteen. the results of the assessment of the behavior of food handlers amounted to 8 people, there were 4 (50%) respondents who behaved less well, 4 (50%) respondents behaved quite well and no respondents behaved very well. Bad behaviors include not washing hands after defecating, not using PPE such as headgear and aprons, and smoking behavior. And there is still a lack of knowledge of handlers about healthy canteens, and knowledge related to sanitation and hygiene. The quadrant position for the healthy canteen management strategy at the UNRI campus based on the IFAS value of 1.33 and the EFAS value of 1.23 lies in quadrant I or the SO strategy. SO strategy is an aggressive strategy that uses internal strengths to take advantage of external opportunities. The strategy that can be applied is by conducting regular canteen supervision and improving healthy canteen policy soup and personal hygiene supervision for food handlers. Based on the research that has been done regarding the management of healthy canteens on the UNRI Campus, several things can be suggested, namely. (1) for further researchers, it is necessary to carry out further studies related to food equipment, food ingredients, and healthy food presentation in realizing a healthy canteen on the UNRI campus, (2) it is necessary to strengthen the healthy canteen policy on the UNRI campus. (3) the canteen owner is expected to improve the hygiene and sanitation of the canteen more optimally by increasing healthy behavior, especially for food handlers.
